Medicare
How do I get CDM Program referral?
The CDM program will be provided by your GP if you are deemed eligible for this service following a clinical judgement from the GP after considering a patient’s medical condition and healthcare needs. This program allows for a maximum of 5 Allied Health Service rebates under Medicare per year.
What do I do once I receive the referral?
Once you have received your CDM referral from your GP, you can contact our clinic to make your appointment. You will need to mention you have a CDM plan when making your appointment so the appropriate coding can be used.
How much will I need to pay?
You may be eligible for:
- Up to 5x 30-minute consultations with our Physiotherapists
- Medicare will provide a $60.00 rebate after full payment of service

DVA
We accept DVA Gold or White card for full payment of our services and will bill DVA directly for any care provided to you under DVA arrangements. This will ensure you do not incur any out pocket expenses.

Workers Compensation
Please bring along to your initial consultation:
- Claim acceptance letter from your relevant insuring company from work
- Claim details – including claim number, case manager name and contact details
- GP referral letter
Who is responsible for the bills?
We will liaise with your case manager from the relevant insurance company to seek approval of physiotherapy treatment plans under the WorkCoverWA rates. Once approval has been granted, all fees will be directed to the insuring company with no out of pocket expense for clients. However, any treatment outside of the proposed plan will be the responsibility of the client.

Motor Vehicle Accident
Please bring along to your initial consultation:
- Your general practitioner referral
- Your CTP (Compulsory Third Party) letter of acceptance with a claim number
- Your insurance company details including case manager name, contact details (email, address, phone number) and billing details
- Any relevant tests or scans
Who is responsible for bills?
We will liaise with the Insurance Commission of Western Australia (ICWA) to seek approval of relevant physiotherapy treatment plans. Once approval has been granted, all fees will be directed to ICWA with no out of pocket expense for clients. However, any treatment outside of the proposed plan will be the responsibility of the client.
